El Dorado St., Stockton CA <br /> Our office is still missing the following documents from the recent change of ownership <br /> for the Underground Storage Tanks (UST) for the above referenced facility. Please <br /> complete the following items: <br /> 1. Underground Storage Tanks—Tank Pages—Form B— Sign and date the bottom <br /> of each form. Two pages for each tank. Provide copies of these forms to our <br /> office. <br /> 2. Information and Instructions for Obtaining A Permanent California ID Number: <br /> Fill out the application for a California Hazardous Waste Permanent ID Number <br /> and fax it to (916) 255-4703. Provide a copy of the completed application. <br /> 3. Certification of Financial Responsibility/Letter from Chief Financial Officer: Fill <br /> out the form provided and make a copy for our records. This document is <br /> required to be submitted annually. <br /> 4. Underground Storage Tank Monitoring and Response Plan: Fill out and submit a <br /> copy. Retain onsite for future reference by employees. <br /> 5.
