The following is an itemized list of hazardous materials violations that have not been <br /> addressed for EI Dorado Gas & Mart as of June 24, 2019. <br /> Open violations from May 01, 2019 inspection <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not show four(4) storm drains located on the property, carbon dioxide gas handling and storage <br /> areas, and some of the codes/symbols used in the map are not included in the legends. A site map shall contain a <br /> north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Violation#9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually by January 15 for 2018 and there has been no submittal for 2019. The hazardous <br /> materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning <br /> November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or <br /> updated information, and submit to the EHD for approval. <br /> Violation#12 -Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review. <br /> Page 1 of 1 <br />
