S A N IJ O A Q U IN Environmental Health Department <br /> COUNTY— <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FAMILY DOLLAR#31867 2318 S AIRPORT WAY STOCKTON Aril 25 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> they would be placed in a hazardous waste tote,except she was unsure which tote they should be placed in.When <br /> Gloria scanned the items,the hand held scanner did not provide which tote the items should be placed in. Gloria <br /> stated she thought the fabric softener should go into an OX HW tote since she believed it contained Clorox bleach. <br /> However, nowhere on the container was the word"bleach"labeled. Bleach was not included in the ingredient list on <br /> the container. Gloria still was unable to make a hazardous waste determination. Gloria seemed unaware of or could <br /> not remember to utilize the Family Dollar Material Safety Data Sheet(MSDS)computer portal or the phone number to <br /> call in case questions came up, to assist her in making an accurate waste determination. It was not until the <br /> inspector mentioned the Family Dollar MSDS portal did Gloria state she would try to find the Clorox Fraganzia fabric <br /> softener SDS. Gloria stated based on her findings from the item's SDS, she would place the fabric softener into an OX <br /> HW tote. <br /> -Two containers of Ampro Pro Styl protein styling gel were found in a cardboard box behind a register at the front of <br /> the facility. One of the containers was broken and leaking, contaminating the box and the second container(IMG <br /> 5600-5603). Later,the inspector found the contaminated cardboard box and the containers of Ampro Pro Style protein <br /> styling gel in a trash can located behind the register(IMG 5630, 5632).When asked who placed the items into the <br /> trash, Monica stated she did not know.When the containers of gel were scanned, the hand held scanner indicated the <br /> gel should be placed in a ST HW tote(IMG 5636). Monica removed the two containers of gel and the contaminated <br /> cardboard box from the trash and took them to the HW storage area. <br /> -A full bottle of Body Fantasies Coconut Fragrance Body Spray was found missing the actuator and in a trash can <br /> located behind a register at the front of the facility(IMG 5630-1, 5633). Monica did not know who placed the item in <br /> the trash. The hand held scanner indicated Body Fantasies body spray should be placed in the AF HW tote(IMG <br /> 5634). Monica removed the body spray from the trash and took it to the HW storage area. <br /> -A full bottle of Modesa Vitamin Moisturizing Skin Oil was found in a trash can located behind a register at the front of <br /> the facility(IMG 5630-1, 5633). Monica did not know who placed the item in the trash.The hand held scanner <br /> indicated the Modesa skin oil should be placed in the CA HW tote(IMG 5635). Monica removed the skin oil from the <br /> trash and took it to the HW storage area. <br /> -A Glade aerosol can was found missing it's actuator, behind a register at the front of the facility(IMG 5604). Monica <br /> did not know how long the aerosol can had been there.When the item was scanned,the hand held scanner indicated <br /> the item should be placed in a ST HW tote(IMG 5608-10). Monica placed the aerosol can into a bag and into an AF <br /> HW tote at the time of inspection. <br /> -A sales shelf was found with spillage of what appeared to be Ajax with Bleach powder cleaner(IMG 5606).The <br /> spillage was also on the floor in front of the shelf.The sales tag on the shelf read"Ajax powder cleanser 28oz <br /> 14-12099 05364"(IMG 5605).The spill was cleaned by a store employee, placed in a bag, labeled the bag"ST,"and <br /> placed the bag into an ST HW tote at the time of inspection. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. <br /> Provide proof of training and training materials to the EHD for employees whose responsibilities include hazardous <br /> waste. <br /> FA0001729 PR0544373 SCO01 04/25/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 7 of 16 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
