SAN J O h Q U I N Environmental Health Department <br /> COUNTY— Int <br /> a"'" Time In: 2:33 pm <br /> `OF.'R �r£Otn SS grows h�r4. <br /> Time Out: 2:57 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: MCDONALDS#24092 Date: 12/26/2017 <br /> Address: 2415 W HAMMER LN, STOCKTON 95209 <br /> Owner/Operator: CRAIG SCHRADER Telephone: (209)954-0502 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00045302 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> 12/19/2017 COMPLAINANT ORDERED A BURGER AND THEY NOTICED THE BOX WAS TORN.WHEN THEY OPENED <br /> THE BOX CONTAINING THE BURGER THEY NOTICED THERE IS A FOOTPRINT INSIDE OF THE BURGER BOX. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I spoke with manager who was aware of complaint prior to my visit. <br /> Per manager, she looked through hamburger boxes and did not observe tears or footprints on boxes. <br /> Per manager, if box is torn or if box falls to the ground, box is discarded. <br /> I observed where hamburger boxes are stored on back storage shelf. <br /> Boxes come to the facility in sealed plastic bag. <br /> Boxes are stored on wire holding rack above Universal Holding Containers at assembly line. <br /> I observed boxes stored at wire holding rack, boxes appear to be intact at this time. <br /> I did not observe tears or footprints at this time. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: liz millan, general manager <br /> EH Specialist: STEPHANIE RAMIREZ Phone: (209)468-9851 <br /> FA0012180 C00045302 SCO04 12/26/2017 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
