SAN J O A Q U I N Environmental Health Department <br /> _._..00UN 1 Y----- _. <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: —7D <br /> FOOD 4 LESS FUEL CENTER` 3408 MANTHEY RD, STOCKTON mber 10,2018 <br /> Item# Remarks <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Maintenance and monitoring records for the last three years were not found on site. A work order listed by the <br /> designated operator for the L5-87 STP for 1/25/18 alarm was not found on site. These records shall be maintained <br /> on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling;(2) <br /> monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of <br /> sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, <br /> ground water elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately <br /> locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the <br /> EHD. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document the following: <br /> On the 10/30/18, and 11/27/18 designated operator(DO)inspection report under <br /> -Section III <br /> The DO failed to add a description to all the components marked N/A in section VII through XI. <br /> -Section IX <br /> The DO failed to mark Yes or No to the question"Are all sensors in under-dispenser containment located to detect a <br /> leak at the earliest opportunity?" <br /> -Section X <br /> The DO failed to answer the following questions"Has the tank tightness testing been completed within required time <br /> frames?"and"Has the line tightness testing been completed within the required time frames?' <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> inspections at least once every 30 days at this facility and are including all of the required information on the reports <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d)Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The site has flexible piping as primary and secondary containment. During the routine inspection, it was noted that <br /> shrader valves in 87 product and 91 product STP sumps were not properly positioned. Furthermore in under <br /> dispenser containment sump(UDC) 1/2, 3/4 the test boots were not removed nor were the schrader valve properly <br /> positioned for the 91 product and 87 product. Lastly in UDC 9/10,7/8 the 87 product line test boots were not <br /> removed nor were the schrader valve properly positioned. Any potential release from the primary containment into <br /> the secondary containment in the 87 product and 91 product piping does not flow into a collection sump. All <br /> secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the low <br /> point of the underground piping.The service technician on site was able to move the Schrader valves to the right <br /> location in the 87 and 91 StP sump.
