VICTOR ROAD, LODI 95240 <br />We are discontinuing review of your work plan because it is incomplete or missing <br />data. We cannot properly review it until the deficiencies checked below are <br />corrected. Please complete the following: <br />Results of previous investigations. <br />The stamp or seal of a registered geologist or certified <br />California geologist, or registered engineer (State of California <br />registration). <br />Detailed site map showing tank location and type, piping, streets <br />and adjacent properties (north toward the top of the page). <br />Results of tank tests (including type of test). <br />Estimates of quantity and compositions of the liquid leaked <br />(including basis of estimate). <br />History of site use (including tank use). <br />Local topography, geology, nearby streams, estimated or measured <br />depth to groundwater. <br />Location, depth, and type (municipal, domestic, irrigation) of <br />water wells within 500 feet. <br />Location of nearby septic tanks, leachfields, buildings, and <br />underground public utility lines (including water, sanitary sewer <br />and storm sewer). <br />X Proposed program, including methodology, to define the extent of <br />soil and groundwater contamination. <br />Proposal to dispose of soil boring(s) cuttings and/or bailed water <br />volumes from monitoring well(s). <br />Lithologic logs have not been signed by the registered geologist <br />or certified engineering geologist (State of Calif. registration) <br />who is responsible for logs' accuracy. <br />Depths to groundwater; shallow, unconfined and confined zones. <br />Elevations of monitor wells and/or domestic well(s) on property. <br />Groundwater gradients based on periodic water level measurements <br />from on-site monitor wells and domestic well(s). <br />Lithology and cross sections based on boreholes, trenches, and <br />other supporting information. <br />X Proposal for additional boreholes and/or monitor wells necessary <br />to define the extent of contamination. <br />X Soil Groundwater <br />X Other See attached Regional Board comments. <br />EH N 45\CHECLIST <br />
