PR 3 �� RECEIVED <br /> JUN 12 2019 <br /> U:NN'IRONNIENTA L HEALTH <br /> June 10, 2019 Dh:NARTNIENT <br /> RE: Environmental Health Department Letter for Compliance <br /> Leyna Huynh, <br /> Due to our place of business being destroyed by fire on May 9, 2019. 1 am providing a written statement <br /> that all of the employees at Butler Environmental Cleaning Services were trained in all the required <br /> areas and elements required by the Environmental Health Department. They were initially trained <br /> during their orientation prior to starting employment and they attended safety meetings monthly no <br /> less than quarterly in which all our employees were trained on issues and concerns to Haz Mat and all <br /> hazards (MSDS, SDS) including the emergency evacuation and response procedures. <br /> We maintained a hazardous materials inventory binder and the business plan, on site at all times while <br /> we operated business.
