The following is an itemized list of aboveground petroleum storage act violations that <br /> have not been addressed for NORCAL TRANSPORT REFRIGERATION as of July <br /> 09, 2019. <br /> Open violations from May 09, 2019 inspection <br /> Violation#4010 - Unlisted Administration/Documentation violation. <br /> The Substantial Harm Criteria certification form was not included with the SPCC plan. If the facility does not meet <br /> the substantial harm criteria listed in Attachment C-1 to this appendix, the owner or operator shall complete and <br /> maintain at the facility the certification form contained in Attachment C-II to this appendix. In the event an alternative <br /> formula that is comparable to the one in this appendix is used to evaluate the substantial harm criteria, the owner or <br /> operator shall attach documentation to the certification form that demonstrates the reliability and analytical <br /> soundness of the comparable formula and shall notify the Regional Administrator in writing that an alternative <br /> formula was used. Ensure that the SPCC plan contains the Substantial Harm Criteria certification form. Submit a <br /> copy to the EHD. <br /> Page 2 of 2 <br />
