The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Muniappa Naidu at (209) 468-3439 to arrange for the follwing inspection <br /> appointment—a 48 hour notice is required: <br /> a. Functionality test to ensure that monitoring has been restored to meet the <br /> requirements of California Code of Regulations Title 23 and the Health& <br /> Safety Code. <br /> 3. Please be aware that the facility cannot operate before the EHD inspection and <br /> approval. <br /> 4. A current printout from the monitoring panel showing system condition PRIOR <br /> TO COLD START must be available for EHD inspection. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board, and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced. <br /> and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br />
