The"Secondary Containment <br /> Test Date,"was incorrectly noted as being performed on 9/7/14.The secondary containment test was performed on <br /> 9/17/14. During the monthly inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on the <br /> reports. <br /> This was corrected onsite. <br /> This is a Class II violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The model 323 sensors in the 89 and 91 submersible turbine pump sumps failed to activate an audible and visual <br /> alarm when tested to ensure continuous monitoring of the tank system. <br /> The technician retested the sensor, and it passed. <br /> This was corrected onsite. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed,analyzed, and properly disposed of(pre-Jul 2003). <br /> Approximately 2 cups liquid was observed in the 89 submersible turbine pump sump and 1/2 cup of liquid was <br /> observed in the 91 spill container. If water could enter into the secondary containment by precipitation or infiltration, <br /> it must be removed and disposed of properly. Ensure that all sumps and annular spaces are maintained free of <br /> liquid. <br /> This was corrected onsite. <br /> This is a Class II violation. <br /> 318 CCR 2636(0(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 89-FE Petro line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br /> detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor <br /> repair or replace the failed leak detector(LG 113-x listed, if applicable)and obtain a permit within one business day <br /> from the EHD. If the failed leak detector can't be replaced immediately,there is a possibility that the 89 grade UST <br /> system may be red tagged to prevent fuel inputs. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Page 4 of 5 <br />
