The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shalt be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a Class II violation. <br /> 12 HSC 25505(c)Failed to have the business plan readily available to facility or EHD personnel, <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map requited by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review, <br /> This was corrected on site, <br /> This is a repeat violation, Class It. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection, January 10, 2018. Lather the <br /> same day, the report was amended by the EHD. This amended report replaces the initial report. <br /> The report was amended to add the narrative to violation 12. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> --documenting the corrective actions that.have-been-or-will-be taken for each violation,and any supporting <br /> paperwork, within 30 days of receiving the completed inspection report. <br /> Facility does not have internal roads and this element was.not addressed on the site map. Please address this <br /> by either writing "NA"for not applicable or a statement that says the element is not on site. Please correct this <br /> by the next submittal. <br /> A 110 gallion diesel tank is used to hold diesel from the 100 gallon fuel tanks attached to the refrigerated <br /> trailers. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
