Contact the fire department and the San Joaquin Valley Air Pollution Control District for applicable <br /> permits. <br /> 2. The contractor shall be licensed by the Contractors Licensing Board. <br /> 3. Any changes shall be submitted to EHD for approval prior to commencing work. All new submittals <br /> supercede the previous ones. <br /> 4. A Holiday Test must be performed at the time of installation. <br /> 5. Both primary and secondary pipes shall bear a UL label. <br /> 6. All pipe fittings and connections shall bear a UL label. <br /> 7. The vapor recovery,vent and fill risers shall be secondary contained per plans. <br /> 8. All monitoring systems shall be listed on the LG113-16. <br /> 9. All testing shall be in compliance with manufacturer's instruction, section 2635 and section 2636 of <br /> California Code of Regulations(CCR) <br /> 10. Automatic pump shutdown must satisfy CCR 2636(g). <br /> 11. All piping must slope to a containment sump with an audible/visual alarm. <br /> 12. All tank top openings,that are connected to the primary space of the UST must have secondary <br /> containment,audible/visual alarm and be vapor tight or terminate at grade. <br /> 13. Sumps and under dispenser containment must be constructed in a manner to prevent vapors from <br /> entering the backfill. <br /> 14. All secondary containment must be constructed to prevent water intrusion. <br /> 15. Overfill prevention shall satisfy CCR 2635(b)(2)(B). <br /> 16. Underground storage tanks are to be anchored per the permit application. <br /> 17. Submit employee training certification from the manufacture of the tank and piping prior to <br /> commencing installation. <br /> 18. An accurate and drawn-to scale-as-built plan must be submitted to EHD prior to scheduling a final <br /> inspection. <br /> 19. Passing Enhanced Leak Detection(ELD)Results must be received prior to the final inspection. <br /> Note:A separate permit application is required to install ELD. <br /> 20. Contact Michelle Le, Senior REHS (209)468-9847 for inspections. See inspection checklist for the <br /> required inspections. All appointments must be scheduled 48 hours in advance. <br /> 21. This permit expires on December 31, 2003.
