Craig, Inc. <br />From: Michael Kith, Sr. REHS <br />San Joaquin County <br />Environmental Health Division <br />Re: Application for permit to install new piping at Mason Center LLC, 6425 Pacific <br />Ave., Stockton, Ca. <br />After reviewing your application for permit to install new piping at above location, I <br />observed the following deficiencies: <br />1. Accurate information on Form A and B. On Form A, Lincoln Properties LTD is <br />listed as tank and property owner. On Form B, Mason Center LLC is the owner. <br />2. Certificate of training issued by piping manufacturer. <br />3. Information and dimension of the dispenser sump. Cut -sheet shows Bravo Box, the <br />plan shows something else. <br />4. Overfill containment. The application lists both 5 gallons and 15 gallons. Please <br />specify. <br />5. Information and cut -sheet for flex boots. <br />6. Information and cut -sheet for all sensors and probes. <br />7. Information and cut -sheet for turbine sump. <br />8. Is there striker plate under all tank openings? The plan shows none. Submit accurate <br />cross section diagram of the tank. <br />9. On page 5 of your plan, there is tank setting note. Is tank setting part of this permit? <br />If so, submit information, cut -sheet and certificate of training issued by tank <br />manufacturer to EHD. <br />10. Location of monitoring panel. Submit site plan. <br />11. Location of all existing monitoring well. The site plan shows none. <br />12. Page 6 of the plan shows fiberglass pipe penetrating the ground surface. If this is <br />correct, please submit letter of approval issued by City of Stockton Fire Department. <br />If not, please submit an accurate piping layout. <br />13.
