I need to receive the following to <br />approve it: <br />1. A contractor who has an "A" license or other license listed in Title 23 CCR section 2715(i). Western Fiberglass <br />has a D12 license which is not listed in this section. <br />2. The contractor with the "A" license must also have an ICC UST service technician and Western Fiberglass FRP <br />cuffs training certificates. This person must be onsite supervising when the work is being done. <br />3. 3`d party testing documents for the Western Fiberglass FRP cuffs system including the adhesives.
