In order to <br /> collect repeatable groundwater samples around the underground storage tanks (UST) and the dispenser area, <br />+ PHS-EHD will require you to install at least three (3) shallow groundwater monitoring wells. A work plan r <br /> that details the construction and location for the monitoring wells should be submitted to PHS- <br /> EHD by December 1,1999. <br /> Please submit the (monitoring)Well Permit Application Form,the$89 application fee, and a Site Specific € <br /> Safety Plan along with the work plan. A two-page application is endbi sed for your use. <br /> PHS-EHD has been notified that this site has received a Letter of Commitment from the State Water <br /> Resources Control Board(SWRCB)Clean Up Fund(CUF). Thank you for your continued cooperation and <br /># compliance efforts.
