Charter Way <br /> Stockton, CA <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) <br /> requested selection of active soil and groundwater remediation alternatives for the above <br /> referenced site in a letter dated August 29, 1991 (copy enclosed). To date PHS/EHD has <br /> not received any further reports or been informed of any site activities. <br /> Please be advised that pursuant to Section 2652(d) of the Underground Storage Tank <br /> Regulations (California Code of Regulations, Title 23, Chapter 16) a quarterly <br /> monitoring report shall be submitted to PHS/EHD and Central Valley Regional Water <br /> Quality Control Board (CVRWQCB) until the investigation and cleanup of the site is <br /> deemed adequate. Submission of these reports is to begin immediately. The report is to <br /> show the: <br /> -status of investigation and cleanup activities including the results of all <br /> investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the <br /> completion of the investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and <br /> any contaminated soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, <br /> gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if known, and changes <br /> in the above measurements over the last quarter. <br /> Records indicate that this site has qualified for the initial priority listing under the <br /> "UST Cleanup Fund" dated April 21, 1992. Your ability to qualify for reimbursement <br /> under this program is partly dependent upon your compliance with PHS/EHD. <br /> A Division of San Joaquin County Health Care Services 1( / <br />
