Our records indicate this is the first offense. <br /> Her decision was to make the driller take out a permit for double fees. No hearing is <br /> scheduled. No action with the California Contractors Board for this offense. <br /> attached is the permit application. have the C-57 fill out page one only and ONLY the C- <br /> 57 license holder sign it. <br /> send it to the EHD with a check for $976.
