Now, all permit <br /> applications are to be submitted with a $366 check/electronic payment per parcel and per C-57 <br /> contractor. In addition, we charge a "per well" charge of$122.00 each or a single $122 fee for <br /> ALL borings (not wells) installed per parcel. As an example, since you are only installing <br /> borings, and not wells, on only ONE parcel by ONE contractor, your fees for this phase is <br /> $366 application fee plus $122 fee for the borings, = $488. You are $366 short. <br /> Also needed to be submitted with each application is a completed and signed Master File <br /> Record'MFR' (form PDF attached). <br /> I will be out of the office from August 27 to September 28, 2010. 1 will not be able to process <br /> or issue this permit if you plan on conducting this work in my absence. If you would like to drill <br /> during September 2010, please submit the $366 and the completed and signed MFR to the <br /> EHD, attention Ms. Adrienne Ellsaesser, REHS and include the site address, "1904 W. Charter <br /> Way, Stockton" in your cover letter so she'll know where the money and form need to go. <br /> I have made a note in the file that it is OK to issue the permit when these items have been <br /> 8/26/2010 <br />
