(GSHI), The Source Group, Inc. (SGI) has prepared this <br /> Monitoring Well Destruction Report for the Oxychem Stockton Site located at 1904 West Charter <br /> Way, Stockton, CA. (Site, Figure 1). Three of the Site's monitoring wells (MW-3, MW-4, and MW- <br /> 19) were destroyed as described in SGI's February 11, 2013 Work Plan and following the <br /> conditions outlined in the California Regional Water Quality Control Board (CRWQCB), Central <br /> Valley Region's conditional concurrence letter dated March 6, 2013. The work was performed <br /> under permit from the San Joaquin County Environmental Health Department (SJCEHD). A copy <br /> of the permit is included as Attachment A. <br /> On March 19th and 20'x, 2013, SGI staff directed Woodward Drilling to destroy the 3 monitoring <br /> wells. In accordance with SJCEHD permits, monitoring wells MW-3, MW-4, And MW-19 were <br /> destroyed by removing the well boxes and drilling out all well construction materials. The wells <br /> were then grouted from the bottom up with neat cement to approximately 4 feet below ground <br /> surface, as requested by CRWQCB. Following grouting, the remaining well cavities were <br /> backfilled with soil to match the existing surface grade. <br /> The Department of Water Resources (DWR)Well Completion/Destruction Forms (Form 188)were <br /> submitted to DWR and CRWQCB on April 25, 2013.
