ACCESS AGREEMENT <br /> Monitoring Well Installation <br /> 121 South Cherokee Lane, Lodi, CA. <br /> As the property owner of 12i South Cherokee Lane,I(Mr.Emil Pansky)do hereby grant permission <br /> to Advanced GeoEnvironmental,Inc.(AGE)to install 3 monitoring wells at 121 South Cherokee <br /> Lane, Lodi, CA. The investigation is being performed under the directive of the Central Valley <br /> Regional Water Quality Control Board(CVRWQCB). <br /> The scope of work for investigation at 121 South Cherokee Lane will include drilling three 8.25-inch <br /> diameter borehole utilizing a truck mounted drill rig to approximate depths of 70 feet, 80 feet, and <br /> 120 feet below site ,grade (bs,) and installation of a two-inch diameter. polvvinylchloride (PVC) <br /> ground water monitoring well within each borehole. <br /> Prior to performance of any field operations, a 48 hours notification to Mr. Emil Pansky will be <br /> given. Drilling activities will be completed in approximately two days. T ie area utilized for boring <br /> advancement acti vities at 121. South Cherokee Lane shall be returned to it3 original conditions prior <br /> to drilling activities. <br /> All costs for performance of the subsurface investigation will be the sole responsibility of Geweke <br /> Land Development and Marketing (GLDM).
