SANJ O 1` Q U I `IA,II, Environmental Health Department <br /> COUNTY <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> OLWD WATER TREATMENT PLANT 3789 W WOODWARD AVE, MANTECA May 25, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 15 HSC 25510(a)Failed to report actual or threatened release to the EHD and/or CalOES Warning Center. <br /> A release of Ferric Chloride occurred multiple times throughout the past years witch was not reported to the EHD <br /> and CalOES. I asked Casey Witchert if the business has ever reported the spill or cleaned it up and I was told no <br /> reporting has been made and that you cant clean up the ferric because it stains everything. A release or <br /> threatened release of a hazardous material shall be verbally reported immediately to the EHD and CalOES. Ensure <br /> that any future releases or threatened releases of a hazardous material are verbally reported immediately to the <br /> EHD and CalOES. <br /> This is a Class I violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 6/24/2018. Heavy staining of the ground and equipment from contact with the Ferric Chloride <br /> have been documented by photos taken on 6/25/2018. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br /> information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
