Written Spill Notification <br /> The following information is to be submitted to the Regional Administrator of the EPA Region IX,within <br /> 60 days of applicable oil spills(see Section 4.5.3 of the SPCC Plan for definition of an applicable spill). <br /> 1. Facility: YRC#813 Terminal <br /> 1535 East Pescadero Ave <br /> Tracy, CA 95304 <br /> 2. Owner of the facility: <br /> YRC, Inc. <br /> 3. Address: <br /> 10990 Roe Avenue <br /> Overland Park,KS 66207 <br /> 4. Date and year of initial facility operation: 1991 <br /> 5. Maximum oil or fuel storage or handling capacity of the facility and normal daily oil or <br /> fuel throughput: Storage capacity approximately 163,450alg lons. <br /> 6. Description of the facility,including maps, flow diagrams,and topographical maps. <br /> 7. A complete copy of the SPCC Plan with any amendments. <br /> 8. The cause of spill,including a failure analysis of system or subsystem in which failure <br /> occurred. <br /> 9. The corrective actions and countermeasures taken, including an adequate description of <br /> equipment repairs and replacement. <br /> 10. Additional preventive measures taken or contemplated to minimize the possibility of <br /> recurrence. <br /> 11. Such other information as the Regional Administrator may reasonably require pertinent <br /> to the SPCC Plan or spill event. <br /> A-4 <br />
