Cherokee Rd. 1800-1; 1800-2 ; 1800-710; 1800-89 <br /> Stockton, CA 95208 <br /> The San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) has recently been included in the State Water <br /> Resources Control Board (SWRCB) UST Pilot Program for oversight of <br /> assessment and clean up activities at contaminated underground tank <br /> sites. <br /> In order to expedite the continued field investigative activities <br /> �. and to issue local permits, a Soil Contamination Work Plan should <br /> be submitted to San Joaquin County PHS/EHD which would address the <br /> clean up activities for the following: <br /> a) UAR from Tank #1 pipelines (diesel) - 5/23/88 <br /> b) UAR from Tank #2 pipelines (regular gas) - 4/5/89 <br /> Full system failure - 6/29/89 <br /> C) UAR from tanks #7 and #10 (waste oil) and #8 and #9 <br /> (motor oil) confirmed - 11/20/89 <br /> All proposals and reports submitted must be accompanied by a Letter <br /> of Acknowledgement from the responsible party(ies) which states, at <br /> a minimum, the following: <br /> I (We) declare, under penalty of perjury, that the information <br /> and/or recommendations contained in the attached proposal or <br /> report is/are true and correct, and that all work and reports <br /> which required geologic or engineering evaluations and/or <br /> judgments have been performed under the directions of an <br /> appropriately registered or certified professional. <br /> The attached proposal or report utilizes the "Regional Board <br /> Staff Recommendations For Initial Evaluation and Investigation <br /> of Underground Tanks" and "Appendix All reporting format.
