WASTE DISCHARGE REQUIREMENTS -8- <br /> DEFENSE DEPOT TRACY <br /> SAN JOAQUIN COUNTY <br /> 3. The Discharger shall submit to the Board and the Department of Health <br /> Services the following reports/documents for approval at least two <br /> weeks prior to the commencement of the "Potable Water Startup" phase <br /> of the interim ground water cleanup system: <br /> a) Geologic well logs and well development data sheets for all <br /> extraction, injection, and monitoring wells. <br /> b) The "Installation Data Report" containing a record of the <br /> interim ground water cleanup system as installed, including <br /> completed installation check lists for each component and the <br /> results of the hydrostatic tests conducted on the systems' <br /> pipelines. <br /> c) The "Site Specific Quality Management Plan" (SSQMP) containing <br /> Quality Assurance and Quality Control (QA/QC) procedures <br /> involved in the collection, preservation, packaging, and <br /> transport of samples, field testing, laboratory analyses <br /> ( including the mobile laboratory QA/QC procedures) , record <br /> keeping, data management, and chain-of-custody. <br /> d) Authorized factory representatives' certifications that IRM <br /> components are properly installed. <br /> e) A revised "Startup Schedule" for the startup of the interim <br /> groundwater cleanup system. <br /> 4. The Discharger shall submit to the Board and the Deiartment of Health <br /> Services the "Prove Out Of System" report for app oval at least two <br /> weeks prior to the proposed commencement of full sale operation of <br /> the interim ground water cleanup system. This report shall document <br /> compliance with Discharge Specifications C.1 & C.2 and include a <br /> performance evaluation of the interim groundwater cleanup system, <br /> based upon the data collected during the startup of the interim <br /> system. Full scale operation shall not commence until the Board <br /> determines that the system will meet waste discharge requirements and <br /> that its continued operation is appropriate. <br /> 5. The Discharger shall notify the Board and the Department of Health <br /> Services, within 24 hours, of any shutdown of the interim ground water <br /> system. This notification shall include the cause of the shutdown and <br /> the corrective action taken (or proposed to be take ) to restart the <br /> system. <br /> 6. The Discharger shall notify the Board and the Department of Health <br /> Services, within 24 hours, of any spill associated with the operation <br /> of the interim ground water system. This notification shall include <br /> the size and cause of the spill , any immediate damage to the <br /> environment, and the corrective/cleanup actions taken and/or proposed. <br />
