Third party billing will be accepted by
Reclamation District No. 17 (Dante Nomellini). Our start date has been confirmed for July 11. 1 will coordinate
with you the timing for our inspections, and what exactly you will want to see performed by Precision. Please let
me know if you have any questions. I appreciate your help on this project!
Thanks,
Zac Crawford
ENGEO
From: Mike Infurna [EH] [mailto:MInfurna@sjcehd.com]
Sent: Wednesday, June 29, 20118:45 AM
To: Zac Crawford
Subject: FW: drilling inspection coverage
Zac,
I may be able to transfer the permit IF some conditions are met.
one of them is the "Master File Record" (MFR) form. (attached).
first of all, open the attachment, print it out and fill it out. Identify the "Third Party Billing"
information at the bottom section and get the form to me after the billing party signs the form.
If the billing party remains as "Reclamation District#17- Dante Nomellini" signatory, then it's
possible that I may be able to transfer the permit.
If a NEW billing party is established, we'll have to start all over (new permit/application and
fees).
let's not take months to handle this, ok? I've got dozens of other permits on my desk with
requests for start dates. I'm setting July 7 onward dates now. first 'completed' submittals on
my desk gets confirmed drill dates.
mike.
From: Mike Infurna [EH]
Sent: Tuesday, June 28, 2011 11:04 AM
To: 'Zac Crawford'
Subject: RE: drilling inspection coverage
Zac,
6/29/2011
