Each well head repair is $122. <br /> Typically only 1 or 2 wells are repaired under this type of permit, but with your proposal for 14, <br /> that means lots of money up front. <br /> I can make you a deal IF the following conditions are met: <br /> 1- all repairs done by one C-57 contractor. <br /> 2- all repairs done under one phase of mobilization. as compared to fix a few now, then fix <br /> some next month, a few months later, etc. <br /> 3- 1 approve the work plan that includes the methods, materials, a map, and construction <br /> specifications before the work starts. <br /> 4- provide a valid billing party for all EHD inspection fees over and above the minimum permit <br /> fees I assign. (Submit a signed EHD Master File Record-MFR will do this) <br /> if you can assure me all of these conditions can be met, I can charge you about half of the up <br /> front permit fees (14 x $122) for the permit(s). I'm assuming there are some ONSITE <br /> wells and OFFSITE wells needing repair? correct? if so, I will have to issue separate permits. <br /> (on versus offsite). Each needs it's own fee. I'm thinking four ONSITE and four offsite wells <br /> for EHD fee purposes. This will provide roughly half of the $1708 fees. So submit the work <br /> plan (#3 above) and the details of your proposal and a permit for onsite with $488 and the <br /> same for offsite. <br /> attached is our webpage with the forms you'll need to submit in PDF format. print each out.
