WASTE DISCHARGE REQUIREMAS ORDER NO. 5-01-251 • 17 <br /> CITY OF LATHROP AND CROSSROADS CREA LLC <br /> WASTEWATER TREATMENT PLANT NO. 1 (CROSSROADS),SAN JOAQUIN COUNTY <br /> (unless a specific Provision requires a different type of professional registration). Each report <br /> shall be wet stamped by the registered professional. In addition, the Discharger shall certify <br /> all reports, as required by General Reporting Requirement B.3 of the Standard Provisions. <br /> a. Prior to discharging the wastewater through the new piping system, the Discharger shall <br /> submit a copy of each document listed below (also described in the 5 July 2001 DHS <br /> review of the Title 22 Engineering Report) and shall submit the corresponding DHS <br /> approval letter. <br /> i. Plans and Maps of Domestic and Recycled Water Lines <br /> ii. Use Area Permit and Operations Plan <br /> b. By 11 January 2002, the Discharger shall submit an Operation and Maintenance <br /> (O&M) Plan for the wastewater treatment facility and land application area. The O&M <br /> Plan shall instruct field personnel on how to manage the day-to-day discharge <br /> operations to comply with the terms and conditions of this Order and how to make field <br /> adjustments, as necessary,to preclude nuisance conditions (e.g., standing water and <br /> objectionable odors from ponded wastewater). It shall also include a nuisance condition <br /> troubleshooting flowchart and a description of notification requirements. A copy of the <br /> O&M Plan shall be kept at the facility for reference by operating personnel. Key <br /> personnel shall be familiar with its contents. The O&M Plan shall include the following <br /> documents as report appendices: <br /> i. A Cropping Plan which identifies the land application areas,wastewater application <br /> method,tailwater control method,berms/checks/furrows, crops to be grown, <br /> nitrogen removal calculations, and crop cutting/harvesting/disposal procedures. <br /> ii. A Grading Plan which describes grading activities which will allow application of <br /> wastewater in accordance with the WDRs,particularly Section D, Land Application <br /> Area Specifications. <br /> C. By 11 January 2002,the Discharger shall submit a Sanitary Sewer System Operation, <br /> Maintenance, Overflow Prevention, and Response Plan (SSS Plan)that describes the <br /> actions designed to prevent, or minimize the potential for sanitary sewer overflows. The <br /> Discharger shall maintain the SSS Plan in an up-to-date condition and shall amend the <br /> SSS Plan whenever there is a change (e.g. in the design, construction, operation, or <br /> maintenance of the sanitary sewer system or sewer facilities) that materially affects the <br /> potential for sanitary sewer overflows, or whenever there is a sanitary sewer overflow. <br /> The Discharger shall ensure that the up-to-date SSS Plan is readily available to sewer <br /> system personnel at all times and that sewer system personnel are familiar with it. <br /> i. At a minimum,the Operation and Maintenance portion of the plan shall contain or <br /> describe the following: <br /> 1. Detailed maps of the sanitary sewer system, identifying sewer mains, manholes, <br /> and lift stations; <br />
