The subject site is
located on the west side of the San Joaquin River and south of Old River.
The well to be abandoned was located approximately 1,500 feet north of Stewart Road and
' 2,000 feet east of Cohen Road and was referred to as MW-18 in the referenced report. As can be
seen on the attached well destruction permit, the county records recorded this well as MW-2,
which was the original well number assigned by Roger Foote and Associates in 1993. The
designation MW-18 was more recently assigned by Kleinfelder when they encountered this well
at the subject site; therefore, ENGEO maintained this nomenclature.
The purpose of our services was to confirm that the well destruction was accomplished in
general conformance with the requirements of San Joaquin County Environmental Health
Department.
' The monitoring well was abandoned on July 17, 2006, by using a hydraulic winch and pulley to
remove the full well casing to a depth of approximately 25 feet, and backfilling the resulting hole
with neat cement to approximately 2 feet below the existing ground surface.
Michael Infurna, R.E.H.S. (Senior Registered Environmental Health Specialist) also conducted a
' site visit to observe the well abandonment on behalf of the San Joaquin County Environmental
Health Department (SJCEHD). The attached permit indicates that the county was satisfied with
the procedures and well abandonment. Based on our observations, it is our opinion that the well
' destruction was accomplished in general conformance with the requirements of SJCEHD.
' 580 N.Wilma•Suite A•Ripon,CA 95366•(209)835-0610•Fax(888)279-2698
w ..engeoxom
