i <br /> Mel <br /> k4 KLEINFELDER <br /> On November 26, 2003 monitoring well LMW-3 was reported as damaged and later i <br /> confirmed during subsequent monitoring events (December 10, 2003; December 16, <br /> ' 2003; and January 12, 2004). Monitoring well LMW-3 was first recorded as "Gone" on <br /> March 2, 2004, and presumably destroyed by grading and construction activities in the <br /> area. The former location of the LMW-3 site was west of and within the shoulder area of <br /> Manthey Road and south of what was a driveway for the residence that since has been <br /> t demolished. The installation of LMW-3 was constrained at the time by the proximity of <br /> Manthey Road to the east and the irrigation ditch aligned parallel with Manthey Road to <br /> the west. Historical aerial photographs reviewed from June 2001 shows the turn-out <br /> ' area repeatedly used by field personnel conducting monthly water level measuring tasks. <br /> I <br /> The attached Plate 2 shows the depicted location of LMW-3 on an aerial photograph <br /> taken while the well in place. The well head only measured 4-inch by 4-inch square and <br /> thus cannot be discerned in this photograph at its present scale. Any increased zoom on <br /> the photograph produced less clear results. Presently (August, 2005), the former LMW-3 <br /> site is located within the shoulder area (approximately 10-feet to 12-feet west of the <br /> ' Manthey Road edge) and has experience extensive re-working during grading <br /> operations. The remaining monitoring well casing, if any, is believed to be beneath the <br /> graded soil. Plate 1 shows the location of LMW-3 in relation to the current site <br /> ' configuration. <br /> Monitoring well LMW-5 was last monitored March 30, 2004 and reported as damaged i <br /> during subsequent monitoring events on May 27, 2004; June 30, 2004; July 30, 2004; <br /> ' and, August 20, 2004. The monitoring well was reported as "Gone' on September 27, <br /> 2004. <br /> i' Monitoring well LMW-5 was formerly located on the west side of Manthey Road and I <br /> approximately 200-feet north of the dirt road driveway which led to the residence noted <br /> on Plate 3. The residence has since been demolished, and the soil moving and grading <br /> activities have been performed as part of planned development. The monitoring well, <br /> which existed at the time of the photograph in Plate 3 (June 2001), cannot be discerned <br /> in the photograph. However, a visible turnout located west of Manthey Road delineates <br /> ' the general area where LMW-5 was formerly located. The precise location of LMW-5 <br /> can be estimated from the following historical facts - the placement of LMW-5 was <br /> constrained by Manthey Road to the east, and to the west by farming operations <br /> conducted at the time of well installation. Presently (August, 2005), agricultural disposal <br /> field 52a is also located west of Manthey Road and in the vicinity of the former <br /> monitoring well LMW-5 site. Kleinfelder representatives recently measured a distance of <br /> 20-feet between the western edge of the present-day Manthey Road white line to the toe <br /> ' of the newly constructed soil berm surrounding the disposal field 52a. The berm width is <br /> an additional 13-feet west of Manthey Road. The total distance separating the west <br /> edge of Manthey Road and the easternmost edge of Field 52a is therefore 33-feet. <br /> tThe estimated location of LMW-5 was approximately 10-feet to 12-feet west of Manthey <br /> Road; and therefore outside the agricultural disposal field (approximately 23-feet east of <br /> ' field 52a). The shoulder to Manthey Road has experienced reworking with construction <br /> activities, and the soil berm constructed for field 52a has likely buried any remnant PVC <br /> casing from former monitoring well LMW-5.
