Tony,
am confused as to the moneys sent and the work to be performed.
You seem to have sent$864 for the MFR fee, the inspection fee and the boring permit fee. (Jacquelyn
Lanzon--MasterCard XXX8823)
Our rates went up about a year ago to$125 per hour. The charges for these fees would now be $875.
The MFR fee is$375, the inspection activities fee is$375 and the boring/permit fee is$125.
You also sent a$500 payment. (Anthony Smith-Visa XXX9852). This seems to be for additional fees. I do
not know what these fees are for. (Note: when we originally talked on the phone I was not aware that a new
Master File Record would be required, but itis due to the changes from the previous one).
I only see one permit application. All the work seems to be done on the same parcel.
If all the work is on the same parcel and there is nothing left in the borings so there are no wells(for
measuring later), there only needs to be one permit. If you are applying for 2 permits, let me know what the
second one is for.
The total amount we would want to receive for the permit would be for,one Master File Record ($375), one
inspection activity fees ($375)and_one boring permit fee ($1`25). This would total.to$875.
If this is correct, if you would send in $375, and we could cash the$500 payment. Then we could return the
$864 fees and we would be square.
Let me know what you want to do.
I can not process the permit until the moneys all balance.
Johnny Yoakum
Senior REHS
San Joaquin County
Environmental Health Department
From:Tony Smith [mailto:TSmith@Geosyntec.com]
Sent:Wednesday,July 18,2012 9:56 AM
To:John Yoakum[EH]
Cc:Carolyn Kneiblher
Subject: RE: Submission of boring permit application and supporting materials
Mr.Yoakum,
The attached Boring Permit Application for 800 W Church St in Stockton contains the following.sections:
1.
