The work plan is approved with the following conditions: <br /> 1) Soil borings for the two dual-complete monitoring wells will be advanced using a 12-inch <br /> diameter hollow-stem auger instead of the proposed 10-inch diameter hollow-stem auger. <br /> 2) Selection of soil samples for analyses must include PHS/EHD comment and concurrence. <br /> 3) A well permit application and $89 permit fee must be submitted and the permit issued before <br /> commencement of work. The previous permit expired on 29 April 1998. <br /> PHS/EHD has also reviewed "Quarterly Monitoring Report - February 1998" dated 29 April 1998 <br /> prepared by Advanced GeoEnviron mental, Inc. <br /> The direction of the ground water gradient has changed 156 degrees, swinging from N 200 E in <br /> November 1997 to S 140 E in February 1998. Since May 1997, it has changed 173 degrees. With <br /> the exception of the monitoring event in March 1995 when it was at S 75°W, the direction of the <br /> ground water flow had been consistently north-northwestward. Such drastic changes in the <br /> direction of ground water flow may influence the contamination plume and decisions on <br /> remediation in the future. PHS/EHD recommends a well survey be performed locating domestic, <br /> municipal, agricultural, industrial and other wells within 2000 feet of the site.
