San Joaquin County <br /> Environmental Health Department <br /> N: a X <br /> 1868 East Hazelton Avenue, Stockton, CA 95205-6232 <br /> • c4.. iP• Telephone: (209)468-3420 Fax:(209)464-0138 Web:www.sigov.org/ehd <br /> �IFORN <br /> Food Program Complaint Inspection Report <br /> Name of Facility: VELVET GRILL&CREAMERY Date: 03/07/2017 <br /> Address: 1421 S HAM LN, LODI 95240 <br /> Owner/Operator: VELVET RESTAURANTS INC Telephone: (209)369-8231 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00043050 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THEY TOOK THEIR FAMILY TO FACILITY LAST WEEKEND AND OBSERVED THAT THE <br /> TABLES AND CHAIRS WERE FILTHY, UTENSILS WERE FILTHY AND HAD FRESH CRUD ON THEM, HIGH CHAIR <br /> HAD BLACK SCUM AND CAUSED A RASH ON THEIR CHILD'S LEGS, PANCAKES WERE SOUR, MENS RESTROOM <br /> LOOKED AS IF IT HAS NOT BEEN CLEANED FOR A WHILE. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I spoke to Person In Charge who was not aware of complaint prior to my visit. <br /> I observed Men's and Women's restrooms that appear clean at this time. <br /> I observed seating area, tables, booster chair and high chairs <br /> I observed debris on some high chairs, booster seat and few seating area bench cushions. <br /> Per person in charge these areas are cleaned daily and as needed. <br /> Detail cleaning is needed of these areas. <br /> I observed a cleaning log that has a deep cleaning schedule. <br /> Flooring near bathroom area is sticky. Per person in charge flooring is mopped daily. Detail cleaning needed. <br /> I observed debris on flooring under tables and on a few table legs. Detail clean. <br /> Tabletops are clean, however the perimeter edges of tables have debris.
