The eyewash solution at this facility was observed empty with an expiration date of February 2006. <br /> All facility communications and alarm systems, fire protection equipment, spill control equipment, and <br /> decontamination equipment, shall be tested and maintained as necessary to assure its proper operation <br /> in time of emergency. Immediately conduct all necessary testing and maintenance to ensure all <br /> emergency equipment is properly functioning at all times. <br /> 812. Copies of universal waste fluorescent tube disposal records were not found on site. According to <br /> the employees on site, the fluorescent tubes are changed and removed by corporate maintenance. A <br /> record of monthly maintenance lighting checks was posted on site, but there were no records of the <br /> waste removed. A generator of universal waste (alkaline batteries, mercury containing equipment, <br /> fluorescent bulbs, electronic devices, CRTs, aerosol cans) shall keep a record of each shipment of <br /> universal waste on site for at least three years. The record may take the form of a log, invoice, manifest, <br /> bill of lading, or other shipping document, and shall include the following: <br /> - Name & address of destination facility <br /> - Quantity & type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br /> InapecPor.
