The map does not show/address north orientation, internal roads, storm and sewer drains, access and exit points (office and storage trailer), emergency shutoffs, and emergency response equipment. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". This is a repeat violation, Class II. 8 HSC 25505(a)(4) Failed to submit a training program or program is not reasonable or adequate for facility. The emergency response procedures were lacking. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment - Evacuation plans and procedures, including immediate notice, for the site Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System(CERS) annually for 2017, 2018, and 2019. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year.
