(AGE) for the above-referenced site. In the QR, AGE recommended
implementing the 11 January 2005 Additional Site Assessment Work Plan Addendum (AWPA);
approved by EHD by letter dated 15 April 2005, as soon as the necessary permits have been
acquired. This recommendation had also been included in Quarterly Report— First Quarter 2005,
dated 17 March 2005. EHD had directed that the work be initiated by 21 June 2005.
Four of the proposed CPT borings are located on your site, the fifth is located on the north side of
Clifton Court road on a property owned by Main Stone Corp., who also is the listed owner of your
site. Intuitively, there should be little difficulty obtaining an access agreement to advance the fifth
CPT boring. Please pursue the access agreement and complete the applications for the
necessary drilling permits. Report the dates of contact made in pursuing the access agreement
and permits in the next quarterly monitoring report. One permit will be needed for each parcel.
Submit the well installation permit to this office and initiate the approved scope of work no later
than 01 September 2005.
Please note that effective 01 July 2005, the $89.00 permit application fee must be accompanied
by a three-hour inspection fee of$279.00, for a total of$368.00 for each permit.
Questions or comments may be directed to Nuel Henderson at (209) 468-3436.
Donna Heran, RENS, Director
Environmental Health Division
Nuel C.
