Provide a copy of the Fire Department (District) Permit to Public Health <br /> Services/Environmental Health Division (PHS/EHD) . <br /> 2. Provide a plot plan showing the name of <br /> prostored,erty owner, <br /> faoflabandorunent <br /> ity <br /> address, parcel number, tank size, pro <br /> and tank(s) distance to all wells within 1,000 feet of the abandoned <br /> tank(s) . <br /> Alternative #2 <br /> 1. Provide a record of materials purchased to abandon tank(s) . <br /> 2. Provide a plot plan showing the name of <br /> , <br /> property owner, <br /> facility <br /> d of abandonment <br /> address, parcel number, tank size, pro <br /> and tank(s) distance to all wells within 1,000 feet of the abandoned <br /> tank(s) . <br /> Alternative #3 <br /> 1. Provide a signed statement from the property owner stating date tank <br /> abandoned. <br /> 2.
