s <br /> • <br /> LLNL - Site 300 <br /> August 3, 1988 <br /> Page Four <br /> In order for our review process to continue and to enable the San Joaquin Local <br /> Health District to issue an operating permit to this facility: <br /> 1) Quarterly Monitoring Statements shall be submitted for all tanks listed <br /> above that lack such data; <br /> 2) Daily Inventory Reconciliation shall be <br /> r <br /> for all motor vehicle fuel tanks possessing calibratablelmetegsalternative #5 <br /> 3) Those tanks containing fuel but without meters shall utilize alternative #7 <br /> providing the monitoring criterion can be met; <br /> 4) Tanks removed without SJLHD's permit and inspection shall have closure <br /> information submitted to our department for review for adequacy and compliance <br /> to the Underground Storage Tank Law, Title 23, CCR; and <br /> 5) Fees for 1988 permiting shall be submitted; a breakdown is enclosed. <br /> Enclosed are application forms for completion and submission to our department.
