After hours contact <br /> numbers for CE are (510) 774-3349, (510) 636-8500, or (408) 725-0130. <br /> 12.0 Accident Investigation <br /> All accidents that result in injury to workers, regardless of their nature, shall be reported <br /> to the CE home office and investigated. Documentation of the incident shall be <br /> completed as soon as possible so that measures to prevent a reoccurrence can be <br /> implemented. <br /> In the event of an employee injury, or other serious incident or near miss, this plan shall <br /> be reviewed to determine if changes to work practices, procedures or training need to <br /> be implemented to prevent similar types of incidents. <br /> CE personnel and all CE subcontractors, as applicable, will comply with the provisions <br /> of all applicable CE Standard Operating Procedures and Safe Work Practices, as <br /> mentioned and described in the attached protocols. <br /> Site Specific Safety Plan Prepared By: <br /> Maurice W.
