Comments: <br /> Initial waste tire inspection done on this date and the following was noted: <br /> 1.)Violation PRC §42953 -Waste tire pick up in December of 2016 were hauled by an unregistered waste tire <br /> hauler. <br /> a.)Any person who gives, contracts, or arranges with another person to transport waste or used tires shall utilize <br /> only a person holding a valid waste and used tire hauler registration. <br /> b.) Ensure that the person hauling the waste and used tires is a registered waster tire hauler. <br /> 2.)Violation 14 CCR§ 18459.3-A search done on WTMS from 06/17/2016 to present resulted in 45 manifest.Manifest <br /> observed on WTMS were not on site and available for review during inspection. <br /> a.)Facility shall maintain a copy of the completed manifest form at their place of business. Manifest form shall be retained for 3 <br /> years and be available for review during inspection. <br /> b.)Ensure subsequent manifest are retained for 3 years. <br /> 3.)Violation 14 CCR§ 18460.2- Manifests were not filled out completely and correctly. <br /> a.) The waste tire hauler shall not transport the waste or used tires without a properly completed Manifest Form. <br /> b.) Ensure that all waste tire manifests are filled out completely and correctly before leaving site. <br /> Notes: <br /> 1.) Permission to conduct inspection was granted by Hugo Izaguirre (Owner). <br /> 2.) TPID certificate was available for review. <br /> 3.) Waste tires were counted directly. <br /> 4.)Vehicles with license plates 7060214, 6621673, 8G12871 and decal 17-05145, 17-05144, 17-05143 were on <br /> site during inspection. The vehicles had its decal and corresponding registration certificate. <br /> 5.) Retain all waste tire manifests for 3 years. <br /> 6.) Facility only stores its waste tires at this location. <br /> 7.) Inspection report emailed to hugolzaguirre10@gmail.com. <br /> Re-inspection will occur on or after April 10, 2017. <br />
