Also, your authorization to your consultant, Alisto Engineering, allowed for assorted repairs to the
existing six (6) monitoring wells.
I am writing you today to advise you that some repairs and a site map update are still required. On my recent
field inspection on October 20, 1995, I noticed that the site map your consultant utilizes as well as the map
included:in the quarterly monitoring reports, represents only former conditions and does not accurately display
the site as it exists today.
Enclosedi please find the map being utilized now along with some notations on monitoring wells still in need of
repair and attention. Also, please note the circles with a dot in the center that represent locations of traffic boxes
with unknown purposes containing assorted sizes of pipes and casings inside. A few of these "casings" are not
water-tight and surface liquids are entering the unsealed traffic boxes and migrating down into the casing as
evidenced by the staining and puddling of liquids inside. For clarification I have"highlighted" these boxes that
will require your attention.
Please update the site map and include the existing gas station facility as well as the former one. All traffic
boxes need to identified. All vapor and monitoring well locations are to be labeled and shown also. You may
include this update within the fourth quarterly report. .
Thank you again for your cooperation on the recent repairs. It's my hope that with this request you will be able
to finally, bring this site into compliance with our local codes to preserve and protect the groundwater in our
county.
You may contact me at (209) 468-3454 to schedule any field work or should you have any comments.
Donna Heran, REHS, Director
Environmental Health Divis'
Michael Infurna, Senior REHS Diane M. son, REHS
Site Mitigation Unit Supervisor
enclosure
MI
c: Alisto Engineering - Brady Nagle
A Division of San Joaquin County Health Care Services
