Any discharge of a pollutant that has caused imminent endangerment to <br /> human healthlwelfare, or to the environment and has resulted in the <br /> POTW's use of its emergency authority to halt or prevent such a <br /> discharge. <br /> 2. Violations of compliance schedule_milestones, contained in a local control <br /> mechanism or enforcement order, for starting construction, completing <br /> construction, and. attaining final compliance by 90 days or more after-the <br /> schedule date. <br /> 3. Failure to provide reports for,complianceschedules,, self,-monitoring data, or <br /> categorical standards (baseline monitoring. reports, ..90_. day compliance <br /> reports, and periodic reports) within 30 days from the due date. <br /> 4.
