Peters Claim# 014052 <br /> 2081 Country Club Blvd. <br /> Stockton, CA., 95204 <br /> On May 5, 1999, San Joaquin County Public Health Services,Environmental Health Division (PHS- <br /> EHD) approved the "Work Plan Addendum" dated April 21, 1999 as submitted by Upgradient <br /> Environmental Consultants (UGC). <br /> In phone conversations with Mr. Vic Cherven of UGC,PHS-EHD agreed to allow the responsible <br /> parry time to obtain a Letter of Commitment (LOC) from the State Water Resources Control Board <br /> (SWRCB) Clean Up Fund (CUF) before continuing the site investigation. In addition, it was agreed <br /> that during this time period, steps to secure access agreements and encroachment permits for offsite <br /> work would be taken and PHS-EHD would be notified when the LOC was obtained. The PHS- <br /> EHD well permit application and $89 permit fee was to be submitted when the LOC was obtained. <br /> On August 18, 1999 the SWRCB-CUF issued the Letter of Commitment. As of the date of this <br /> letter,the well permit application and fee and the status report detailing the progress in obtaining the <br /> access agreement/encroachment permits have not been received at this office and the required <br /> subsurface investigation has not been conducted. <br /> A 90-day notification letter was issued by PHS-EHD on November 22, 1999 to Mr. Peters. The site <br /> was allowed until February 23, 2000 to come into compliance with directives issued by PHS-EHD. <br /> On January 20, 2000 San Joaquin County Public Health Services,Environmental Health Division <br /> (PHS-EHD) approved the January 10,2000 "Work Plan Addendum" submitted by Upgradient <br /> Environmental Consulting on January 12,2000 for the placement of additional onsite and offsite <br /> monitoring wells. <br /> On February 3,2000 P1S-EHD approved the "Request to Delay Drilling" dated January 31, 2000 <br /> submitted by UGC on February 2, 2000. The request stipulated that the drilling would begin"late <br /> Febmary or dx begimung qfMarth".
