Please <br /> provide a survey of wells within 2000' of the site and a site map that shows all possible <br /> conduits (i.e., septic fields, sewer, storm drains, telephone, gas, water, etc.). <br /> �.A PHS-EHD is currently holding two boring permit applications and two checks for this <br /> investigation.
