Revised 9-4054 monthly report Page 1 of 3
Mike Infurna [EH]
From : Mike Infurna [EH]
Sent: Tuesday, March 06, 2007 8:31 AM
To: 'Myers, Tonya'
Subject: RE: Revised 9-4054 monthly report
Your permit packet is on my desk. You 're #3 on the "permit issue" list. .
See why 1 recm'd doing this submittal thing 7-14 days AHEAD of your drill schedule. NOW you've got me rushing.
In the future, think of EHD and our turn around time as only slightly quicker than getting a County or City
encroachment permit. . . .you don't expect THEM to issue in less than a week, do you?
From: Myers, Tonya [mailto:Tonya . Myers@arcadis-us.com]
Sent: Monday, March 05, 2007 10 : 54 AM
To: Mike Infurna [EH]
Cc: ndal@chevron .com
Subject: RE: Revised 9-4054 monthly report
Mike,
I sent the boring/well permit and a check for the soil vapor probes on Friday, you should receive it today.
With the permit package I included a site map with the probe locations so however you want to work the
permit is fine. I understand that you spoke with Cascade regarding the temporary soil vapor probes, are
you okay with the set-up? We are still on course to begin this work on Friday (3/9) and continue thru
Saturday (3/10), do these dates still work for you? Cascade will need to hand clear (hand auger) the
boring locations first, so the better half of Friday morning might be spent clearing the locations . We are
only clearing to about 3 feet bgs, so it shouldn't take very long to get the borings cleared . If it works for
you, I will call you on Friday morning after I speak with the driller regarding the sequence of events and
get you a start time .
I will be in the office today, if you have any questions or concerns regarding the schedule or field methods
please let me know.
Thank you
Tonya
From : Mike Infurna [EH] [mailto: MInfurna@sjcehd.com]
Sent: Monday, March 05, 2007 9 : 32 AM
To: Myers, Tonya
Cc: ndal@chevron .com
Subject: RE: Revised 9-4054 monthly report
Tonya, Miguel ,
The letter looks fine and contains all that I asked for. Thank you .
Miguel, have you seen this letter (as per your phone call ) ?
Tonya, are you under the impression you already have a boring/well permit for the ONSITE vapor probe
installations? The last permit for ONSITE work was issued to Cascade by me back on August 4 , 2006 for
MW-1 , MW-2, and MW-4 cluster installations (SR#0047686 ) . NO vapor probe details were included on
3/7/2007
