Boulevard Auto page 2 <br /> 2151 Country Club, Stockton. <br /> A monitoring well is proposed south of the site in Country Club Boulevard. This area is <br /> located at the borderline of City of Stockton and San Joaquin County and you will need <br /> to verify which agency controls the right of way (ROW). An encroachment permit, and <br /> possibly a revocable permit, is required to be on file with PHS-EHD before the boring <br /> permit can be issued. Please contact Mr. Louie Perez, San Joaquin County Public <br /> Works at (209) 468-3057 or Mr. Jason Enders, City of Stockton Permit Administration, <br /> at (209) 937-8349 to verify the ownership of the right of way where the well is proposed <br /> to be located and obtain the permit(s). <br /> In addition to the off-site boring permit application and fee, the borings proposed on-site <br /> will require an additional and separate boring permit application and separate $89 fee. <br /> All ROW permits, applications, and fees should be submitted with the addendum <br /> revision due at PHS-EHD by December 1, 1998. <br /> Please submit a schedule of field activities dates to PHS-EHD along with the workplan <br /> addendum revision. The schedule should detail dates that the plan will be submitted to <br /> the State Water Resource Control Board (SWRCB), Clean Up Fund (CUF) for pre- <br /> approval of costs. The last work plan for this site was approved by PHS-EHD in July, <br /> 1996 and the report of findings was not submitted until a full year later. The elevated <br /> levels of petroleum detected in the shallow groundwater during the Geoprobe <br /> placements of 1996 need to be confirmed and monitored on a regular basis. It is <br /> important that the monitoring wells are installed immediately and the quarterly <br /> monitoring start this year. The schedule is to also include the date the report of findings <br /> is to be submitted to PHS-EHD.
