PHS-
EHD has filed a Notification of Hazardous Waste Discharge as mandated by California Health &Safety Code
section 25180.7. In addition, this site has been added to the local contaminated database as a contaminated
groundwater site and will remain under the oversight of this agency
In order to verify the extent of the soil and groundwater plumes, PHS-EHD will require a groundwater
investigation to be conducted on and off-site. Monitoring wells should be placed in areas that will delineate the
lateral extent of the groundwater while sampling for petroleum in the soil as they are constructed. Monitoring
wells shall be placed so that up-gradient, down-gradient, and lateral gradient groundwater information may be
obtained, as well as a hydraulic gradient(rate) and direction of the flow of the groundwater.
Adequate soil samples should be analyzed from each boring to provide your consultant with adequate data to
eventually determine the total volume of soil impacted and average concentration of petroleum so that PHS-
EHD will be able to evaluate the current, continuing, and future risk to the groundwater. A survey of all
domestic and other producing wells should be conducted for this site and the results included in one of the
quarterly monitoring reports that will be required by PHS-EHD.
A workplan that addresses the above noted concerns should be submitted to PHS-EHD within ninety (90) days
of the date of this letter.
Please address your submittals to Michael Infuma, Senior REHS, and you may call him at(209)468-3454
should you wish to discuss your site.
Donna Heran, REHS, Director
Environmental Health
/hDDivision
v'
'/iii" ✓Y / ` V
Michael Infuma Jr., Senior REHS Margaretlagorio, REHS
LOP/Site Mitigation Unit IV Supervisor
MI/
c: Tim Cuellar-Advanced GeoEnvironmental, 1803 A March Ln., Stkn. 95207
A Division of San Joaquin County Health Cam Services
