Box 388 • Stockton, CA 95201-038� <br /> FOa�\ <br /> 209/468-3420 <br /> RAY HUEY & JOHN PARKER WED JUN 141995 <br /> 2151 COUNTRY CLUB BLVD <br /> STOCKTON CA 95204 <br /> RE: Boulevard Automotive Service SITE CODE: 2424 <br /> 2151 Country Club Blvd. <br /> Stockton, Ca 95204 <br /> In correspondence dated March 7, 1994 you were notified that the above referenced contaminated site was being <br /> prioritized and that a ranking list would be established for investigation and remediation. <br /> On January 5, 1995 you were notified by certified mail that this site has been assigned to the San Joaquin <br /> County Public Health Services, Environmental Health Division (PHS-EHD) Local UST Oversight Program for <br /> oversight of assessment and clean up activities at this contaminated underground tank site. <br /> PHS-EHD now requires you to start your investigation and recommends that you contact environmental <br /> consultants familiar with the "Leaking Underground Tank Manual" (LUFT) and the "Tri-Regional Board Staff <br /> Guidelines for Preliminary Investigation and Evaluation of UGTs" and begin procedures that will allow you to <br /> submit a workplan. <br /> A workplan that follows Appendix A-Reporting (Tri-Regionals) for initial investigation should be submitted to <br /> PHS-EHD for review by September 29, 1995. <br /> Sites seeking eligibility and reimbursement under the Cleanup Fund are required to obtain at least three <br /> competitive bids and be in compliance with PHS-EHD directives and requirements. <br /> Failure to comply with requirements issued by PHS-EHD will prompt enforcement actions that will disqualify <br /> responsible persons from receiving reimbursement and may result in legal actions. <br /> Please contact Michael Infirm at (209) 468-3454 should you have any questions. <br /> Donna Heran, REHS, Director <br /> Envir/onme�ntall Health Division <br /> Michael Infutna, Senior REHS Diane *son, REHS <br /> Site Mitigation Unit Supervisor <br /> MI <br /> A Division of San Joaquin County Health Care Services <br />
