1000 <br /> Former Boulevard Automotive Page 2 of 2 <br /> 2151 Country Club Blvd July 8, 2014 <br /> Stockton, California 95204 <br /> • calcium, cobalt, copper, iron, lead, magnesium, manganese, mercury, molybdenum, <br /> nickel, potassium, selenium, sliver, sodium, thallium, acetone, tungsten, and <br /> hexachrome by a California certified laboratory; <br /> The EHD tentatively approves the CAP submitted by AGE, contingent on compliance with the <br /> following EHD requirements: <br /> 1. In accordance to the corrective action plan (CAP) approval process, the EHD will post a <br /> public notification of pending CAP approval at this site. The public has 30 days to <br /> express any concerns regarding the proposed remedial method for this site. Fieldwork <br /> can begin once the 30-day public participation period is over and all public concerns <br /> have been addressed and the CAP is approved. A copy of the public notice has been <br /> attached for your records. <br /> 2. A current waste discharge permit from CRWQCB must be obtained before any fieldwork <br /> can begin. <br /> 3. Our records indicate a waste oil tank was removed from this site. Although indicated in <br /> the narrative portion of the CAP report, it was not shown on any of the report figures. <br /> Submit a site plan showing the former location of the waste oil tank and please include <br /> the former waste oil tank location on site plans in future reports submitted to EHD. <br /> 4. Notify EHD of all scheduled injection events along with any field work following initial <br /> well installation. <br /> The EHD will issue the permits for the installation of the sixteen injection wells once the <br /> following have been completed. With these conditions, the EHD approves the work plan <br /> submitted. <br /> A. This site has proposed injection wells located on an adjacent parcel. Please submit a <br /> copy of the access agreement signed by the adjacent parcel owner when submitting <br /> the EHD Well and Boring Permit Application form for these wells. <br /> B. All water used for the purpose of sealing a well must be potable water and not water <br /> that has been displaced from a well that is to be destroyed or from a non-potable source. <br /> In the Semi-Annual Groundwater Monitor Report, Fourth Quarter 2013, AGE recommended the <br /> following: <br /> • Continuation of semi-annual, annual and biennial ground water monitoring at the <br /> Boulevard Automotive Service site. <br /> • Semi-monthly free product monitoring in well MW-4. Removal and documentation of <br /> free product using a disposable polyethylene bailer until floating product is no longer <br /> visible. All waste water will be properly labeled on hazardous waste drums and <br /> contained on site. <br />
