I have issued PHS/EHD Drilling Permit#013527 for the installation of the <br /> additional well. The permit was issued following receipt of the Caltrans Encroachment Permit <br /> #1097-6SV-0361 dated August 1, 1997. <br /> As indicated in PHS/EHD correspondence dated August 27, 1997, all field activities related to <br /> completion of the soil and groundwater investigation and feasibility studies must be <br /> completed by October 31, 1997. A Corrective Action Plan must be submitted by December <br /> 31, 1997. <br /> Groundwater contamination of a nearby domestic well has been evidenced. Remedial action <br /> is required to reduce the concentrations of contaminants migrating off site as well as to restore <br /> the groundwater quality beneath the site. <br /> If you have any questions,please do not hesitate to contact me. <br /> 304 East Weber Avenue,Third Floor, Stockton CA 95202 <br />
