Auawl 1,1907 <br /> At completion of project, permittee shall provide Caltrans Representative with copies of any report <br /> containing drilling and groundwater analysis. <br /> prior to expiration of this permit, Permittee shall apply for and obtain a maintenance permit to maintain, <br /> monitor, and abandon the wells Installed under this permit In accordance with Standard Water Quality <br /> Control Board Standards. The maintenance permit may be obtained for a period not to exceed five years <br /> from expiration date of the installation permit. A deposit in the amount of $280 shall be submitted with <br /> the application for said permit. <br /> FIVE WORKING DAYS PRIOR to the start of this work, permittee shall notify the State representative <br /> MIKE HONMA, P,O. BOX 2048, Stockton, CA 95201, (209) 948-3656. All work under this permit must meet <br /> with the approval of the State representative. Permittee shall coordinate with Caltrans Electrical Personnel <br /> FIVE WORKING DAYS prior to drilling so that existing signal conduit can be located. Permittee shall also <br /> coordinate with the landscape contractor prior to start of work. <br /> Fallure on the permiltee's part to comply with any provision will be cause for revocation of this permit. <br /> IMMEDIATIiIN PO1d.OWING COM11.L71ON OF WORK PERMITTED 11F,RFIN, ITPMII'ITe SHALL FILI, <br /> OUT AN13 MAII. NOTICE OF COMPLETION PROVIDED BY GRANTOR. <br /> Permittee's attention is directed to Section 6, "CONTROI. OF MATERIAIS", of the State Standard <br /> Specifications. Reference to Engineer in the Stale Standard Specifications shall include State representative. <br /> Permittee is directed to Standard Specification Section 7-1.11 Preservation of property, and Business and <br /> Professions Code, Section 8771. Permittee shall physically insppect the work site and locate survey <br /> monuments prior to work commencement. Monuments shall be referenced or reset in accordance with <br /> Business and professions Code. <br /> In addition to the attached General Provisions, the following special provisions are also applicable for <br /> Installation and monitoring of groundwater monitoring wells: <br /> 1. Unless specifically authorized elsewhere in this permit, survey work shall not be performed: <br /> a. On holidays or an afternoon preceding a holiday. <br /> b. In medians. <br /> c. When work requires traffic control on lanes. <br /> 2. Permittee shall furnish, place and maintain required signs, safety equipment, anti traffic control and <br /> warning devices in accordance with the Manual of Traffic Controls published by the Department of <br /> Transportation. <br /> 3. All personnel shall wear hard hats and orange vests, shitts or jackets as appropriate. <br /> 4. Unless specifically authorized In this permit, markings within the right-of-way shall be temporary. <br /> Faint used for markings shall be water soluble and other markers shall be removed upon completion of <br /> work. <br /> 5. Any collected data requested by Caltrans shall be furnished to Caltrans without charge. <br /> 6. Vehicles necessary for work may be parked in State Highway right-of-way off paved shoulders. <br /> Auxiliary support and employee vehicles shall remain outside the right-of-way. <br /> 7.
